To Donate Stock in Certificate Form:
- Each Stock Certificate, unsigned. (If your Certificate is signed please contact the Church Business Office before sending for separate designation instructions.)
- A signed Stock Power and a signed Disclaimer are required for each stock certificate. Forms may be obtained by calling the Church Business Office at 734.414.7777.
- Specify the Stock to be used for "General Fund" OR "Campus Development". Without a specific use, the Stock will be assigned by default to 'General Fund".
- Mail the Stock Certificates in one envelope. In another envelope, mail the Stock Power and Disclaimer forms, Both envelopes are to be ailed to the agent responsible for processing NorhRidge Church Stock activities:

NorthRidge Church will mail to your address-of-record a notification of the stock transfer. The Notification is part of the quarterly "Charitable Giving Statement".
To Donate Stock From Broker-To-Broker
This is called a DTC Transfer. No forms are required.
- Contact your Broker and let them know that you wish to have a DTC Transfer of Stock to NorthRidge Church.
- Instruct your Broker to process specific stock(s) with a targeted transfer date to NorthRidge Church. The Broker required the following information. Please select one of the two accounts used by NorthRidge Church.

Campus Development
- Notify the Church Business Office that a Stock Transfer is in process, after you have contacted your Broker.
- Request your Broker to formally notify the Church Business Office when the transfer has been completed.
